What: Join the North Carolina Department of Health and Human Services (NC DHHS) and Vaya Health to learn about Supported Living, the philosophy behind it and how it can expand and strengthen the capacity to support people with disabilities to live in their own homes. Attendees will hear from NC DHHS members of the Division of Medical Assistance (DMA) and Vaya Health on how to facilitate the transition from inpatient facilities to in-home supported care. Speakers will include:
- Kenneth Bausell, I/DD Manager, DMA
- Trish Farnham, Associate Director, Special Initiatives, DMA
- Steve Strom, NC Money Follows the Person Demonstration Project Director, DMA
- Jesse Smathers, Specialty Populations Clinical Director, Vaya Health
When: Monday, September 25th at 12 PM
Why: This webinar will highlight the basics of Supported Living, what rules define the service, how it is funded and what is the service's overall philosophy. Additionally, it will highlight what supported living concepts are and how to improve their collective capacity to support individuals who utilize the supported living service.
