The NC Council on Developmental Disabilities has awarded funding to The Arc of NC, Easter Seals UCP and the Autism Society of NC for a new project, From Planning to Action: Supporting Integrated, Collaborative Care for People with Intellectual and Developmental Disabilities, that builds on the work of the Medical Health Home initiative.
A core focus of this project is funding for two (2) pilot demonstrations that will be awarded through a competitive application process managed by the project team. The Arc of NC serves as the fiscal agent.
The two (2) pilot demonstrations are as follows:
IDD Health Care Consultation to improve the capacity of primary and community health care providers to treat individuals with I/DD through consultative access to medical and clinical professionals with I/DD expertise.
and
Disability-Informed Health Care Navigation to address the challenges encountered when individuals and families move between complex health care and disability service systems.
